Project ONE Club is seeking to hire a

Medical Case Manager (full time)

Job Summary:


Work with HIV/AIDS clients and their caregivers to help them access services and identify barriers to comprehensive care. Develop alliances with other service providers and facilitate consumer input. Cooperate with funders to keep services relevant and efficient and participate in the agency's Quality Improvement process when necessary.

Specific Duties:

·     Provide medical case management to persons withHIV/AIDS of all social, ethnic and economic backgrounds.

·     Assess client needs for services

·     Assist client to access needed services and make referrals

·     Coordinate services for the client

·     Complete documentation of client record in a timely fashion

·     Be vigilant of client confidentiality

·     Provide medical case management services on-site at the Brandywine Community Health Services office in Coatesville

·     Meet with clients in the outpost offices ofChester County in Phoenixville, West Chester, Kennett Square and Oxford

·     Make home visits when needed

·     Address the need for services of the client population of the ChesterCounty prison

·     Define barriers to services and treatment

·     Report barriers which hinder clients' access to services to the Quality Improvement Committee

·     Participate in resource developments for clients

·     Provides advocacy for services and raise public awareness

·     Complete emergency needs applications

·     Provide assistance with emergency needs application

·     Provide financial counseling if needed

·     Provide services to clients in their own environment

·     Enter information into the CareWare System to meet the standards of the funding sources

·     Perform other duties as assigned‍

Qualifications:

The candidate for the position of the HIV/AIDS Medical Case Manager will have a BS or BA degree as a minimum from an accredited college or university or a BSW from an accredited CSWE program. Experience providing direct service to people living with HIV/AIDS is a plus. A master's degree in social work is preferred.

Other Requirements:Practicum or work experience in health, welfare, or social service organization, or equivalent in supervised volunteer experience is helpful.Candidate must have knowledge of HIV/AIDS disease and its treatment, be skilled in relating to minorities and clients of same and bisexual orientation. The MCM needs organizational and data entry skills to meet the documentation and reporting requirements of the funding sources. The position requires professional confidentiality

The HIV/AIDS MCM must have demonstrated his or her ability to relate to and toengage persons in a meaningful way. He/she must be able to use supervision andthe consultation of professionals from social work and other disciplines. TheMCM must have the ability to observe, ask questions and to formulate and carryout with clients a treatment plan, He/she must have persistence, demonstrate a high degree of self-awareness and handle contacts with clients and colleagues in a professional manner. The MCM must be able to carry out administrative aspects of services to the HIV/AIDS clients and handle appropriately the intrinsic authority of his or her job.  They are a representative of the agency to the larger community and must be clearly Identified with the agency’s purpose, goals and methods.
